    Initial admission form

    We strongly suggest that students remain at their current school until the end of their current term.
    Students must be full-time residents of Minnesota to qualify for free, public education. Residence in Minnesota is defined as a student being domiciled in the residence.
    ** Please ensure that you continue attendance at your current school placement while this application is being processed; this process may take up to two weeks.

    ​Once we receive this Initial Application Form and determine if there is space available to enroll you/your student, we will send an Enrollment Packet via email. The information in the enrollment packet must be completed, CCS Online will request records from your current school, and we will contact you to determine classes and to advise you about how to do school at CCS Online.

    Once we have offered you a spot at CCS Online by providing you with the Enrollment Packet, you have 7 calendar days in which to complete the enrollment packet. If we do not receive the enrollment packet within that time, we may give the student’s enrollment position at CCS Online to the next student in line.
